well, attachments are stored as objects in the notes database, so yes, you can read and modify them without touching the document they are attached to.

I have not done this though. One thing - I think you might have problems is with compressed LZW attachments. I've looked at this and it's not clear how you can uncompress them and then compress back. Though you might store back uncompressed version of course, provided you manage to uncompress in the first place.
